Forms Required For Tutoring

Please make sure you understand the description of duties for substitutes and tutors.  This form must be filled out every time you are filling a position that is paid for from state or federal funds.

In this section, you will find the student sign-in sheets, which are required daily and to be signed by students.  Prior to any tutoring the tutor must complete the supplemental service agreement, the principal must sign the agreement and the document forwarded to the External Programs department for approval.
